Thomas (TJ) Carlson is an experienced RN on 3E who has cared for COVID+ patients throughout the pandemic. In January, TJ cared for a veteran with severe COPD and COVID. This veteran has maxed out on BiPap for several days and had a feeding tube as he was unable to maintain his oxygenation when he removed his mask to eat. On this day, he expressed his desire to withdraw cares and very clearly stated that he was “done.”

TJ very gently navigated this emotional situation with the family and provided excellent closed-loop communication with the medicine team, nursing leadership, patient, and family. He honored the veterans wishes to “eat one more shrimp dinner” before passing. TJ coordinated with the medicine team, Respiratory Therapy, and pharmacy to maximize the patient’s oxygenation and comfort in order to reach his goal of eating. When the family brought the meal, TJ pre-medicated the patient and applied a different respiratory device that allowed him to take a few bites before withdrawing cares. The family commended TJ for honoring this wish that may have seemed small to others but meant the world to the patient. The palliative doctor, hospice resource nurse, and 3E nursing staff recognized TJ for his diligent, thoughtful care in this veteran’s final hours.
