Tiffany and I shared a patient recently who came in with a serious complication of a life-long disease. It became apparent that this patient was overwhelmed, emotionally spent, and hadn't kept up as well with recent advances in home management of her disease. In other words, she was "noncompliant." If this patient's behavior had been different, she wouldn't have been in the hospital. Unfortunately, in modern healthcare, it is all too easy to blame the patient and their "noncompliance" for everything. Tiffany took the time to really understand the patient and where she was coming from. She told the patient, "We are going to start from the beginning and go over everything to help you." She quickly got the patient's trust, and uncovered areas that were difficult for the patient. Tiffany involved the rest of the care team, including the pharmacy, health educator, and physician. Tiffany advocated for the patient when the time came to have her transferred from the ICU, advocating for her to be placed in "overflow" status so that the patient would still have very close nursing attention and education. Tiffany approached the problems at hand in an upbeat, collaborative, can-do manner, involving the patient each step of the way. I did not once hear Tiffany utter the word "noncompliant" or shame or blame the patient. I am quite certain that Tiffany's extra patience, time, and caring will make a lasting impact in this patient's life.
