Toni Palmer, RN
Recently, Toni Palmer was recognized by a patient for the care she provided. The patient expressed gratitude for Homecare and collaborative services. She stated she is grateful that she had Covid with pneumonia because she learned people still care about her. She stated that she now knows it's ok to ask for help and no longer feels alone.
To provide a little more background to the situation, the patient had been homeless and recently placed in a subsidized apartment. The patient was without furniture, clothing, food, and other necessities. Toni became her biggest advocate and started reaching out for community support and resources for the patient. Through Toni’s community connections, she was able to get the patient basic furniture, clothing, and food vouchers. The patient shared with the team how grateful she was. She was at a point where she had given up hope but now has a different outlook on life.
