This nurse has gone above and beyond over the past year, from helping out in times of short staffing to using her critical thinking and assessment skills. She has caught near missed events such as a medication discrepancy with a cardiac drug or the deterioration of patients and calling the rapid response team. One of her best attributes though is her ability to advocate for her patients with the most recent event being a few weeks ago. We received a skilled patient to our facility that was nearing the end of life and it was very difficult for this patient's family to accept and come to terms with the reality of the situation.

Multiple attempts were made from different staff members to communicate the importance of end of life care a measure that we could provide this patient during his passing not only to the family but to the provider that was caring for him. This nurse took care of this patient on multiple occasions and recognized that his end of life was approaching rather quickly and was adamant to advocate for him so we could provide him comfortable end of life care. Due to her advocacy not only with the family, but with the provider we were able to transition him to this type of care. If it would not have been for her persistent push this patient would not have been able to pass away peacefully.

Thank you, Tracey, for your exceptional compassion, advocacy, and dedication to your patients. You truly make a difference.
