Where do we begin to describe this RN/Case Manager? Traci Hoskins defines team. She defines patient advocate. She defines friend. The PCU team could not ask for a better Case Manager than this individual. From the time she steps on the unit, to the time she leaves, Traci gives 110% in her role as Case Management. When it is her weekend, and she is covering for the entire facility, everyone witnesses the patient advocacy and energy she brings to the patients, families, and the staff here at Mercy Clermont. She is persistent to meet the discharge needs of all patients she interacts with. Her rapport is outstanding with her departmental peers, nurses, ancillary staff, and above all, the physicians. She truly advocates and will follow up for what is best for the patient. Each one of us, in addition to all patients, are greeted with her infectious smile. Yes, she loves to talk, but communication, great communication, is a defining characteristic I would use to describe her. We appreciate Traci and would love to see her recognized and celebrated for the amazing team member that she is to each one of us!
