Tracy's Patient was to be discharged; however, there was a sudden change in plan. The plan would be to go home instead of the nursing home. The Family was anxious and unsure if they could care for patient. Tracy managed his appointment and care. Tracy went above and beyond by making all appointments for the doctor and outpatient tests, when convenient for family. She also printed directions to those appointments. This patient and family received exceptional care while in the hospital and outside as well due to Tracy's efforts. She demonstrated all the characteristics of a Daisy nurse.
