Tracy Small has consistently demonstrated unparalleled professionalism, exemplary collegiality and exceptional patient care.
A newly diagnosed hypothyroidism patient was discharged with only a seven-day supply of Levothyroxine and instructions to see the endocrinologist within 7 days. However, the endocrinologist's schedule was booked until over a month later. The patient called the Continuum of Care department for assistance because she had no other way of obtaining a prescription to treat her hypothyroidism. The Nurse Care Advisor (NCA), attempted to have the endocrinologist write a script for the mediations needed but the doctor was already gone for the weekend and there was no one covering.
Since the patient did not have a PCP and we were not able to reach the endocrinologist, the NCA contacted Tracy for guidance on how to resolve the patient's issue. Although Tracy was not involved in the direct care of this patient at any point during the patient's hospital stay, she went through the patient's chart, tried to troubleshoot the issue, and ultimately found a concrete resolution for the patient's concerns. The patient was able to obtain a month's supply of her medication, thus allowing the Care Coordination team more time to secure an appt with the endocrinologist.
This is not an isolated incident for Tracy; she constantly goes above and beyond in an effort to assist in the patient's transition from hospital to home. Her ability to make herself available when needed the most makes her a valuable asset to our team and organization.
