Mrs. Odoh personally called multiple times by phone, reaching the Vascular Team, coordinating their arrival at PACU, explaining the purpose of emergent PICC line placement, provide necessary help for its placement in PACU.
A veteran arrived at PACU for elective left knee infected hardware removal and antibiotic spacer placement. On arrival at PACU, it was detected that the existing 20G PIV was not working, and the right forearm was infiltrated. The patient had infected multiple joints, and options on where to get new access for surgery and post-op management were a right arm or placement CVC. Anesthesia service use real-time Ultrasonography for the right upper extremity venous access scan and concluded that the best option for reliable vascular access is PICC line placement.
The patient was updated about the necessity of getting new vascular access to get General Anesthesia during surgery and antibiotics during post-op treatment. PACU nurse Victoria I. Odoh was notified about the vascular access problem and the necessity to reach the Vascular Team for PICC line placement ASAP and coordinate PICC line placement as the patient is expecting orthopedic surgery today, and the Anesthesia Service needs to proceed with another elective surgery timely. Mrs. Odoh immediately recognized the unusual situation and provided reassurance and necessary management to the patient. She also helped the Anesthesia provider how to place a PICC line placement order, as it is not the usual task anesthesia does on a daily basis.
Finally, Mrs. Odoh personally called multiple times by phone, reaching the Vascular Team, coordinating their arrival at PACU, explaining the purpose of emergent PICC line placement, provide necessary help for its placement in PACU. 1) Pre-op placement of a new, less invasive vascular access (PICC line) for surgery and post-op antibiotic treatment
2) Avoidance of CVC placement and unnecessary admission to SICU
3) Allow Anesthesia providers to focus on other veterans' anesthetic management while coordinating PICC line placement with the VA Vascular Team
4) Maintain constant care and support of veterans in stressful perioperative situations.
