Due to Vivian's good critical thinking and nursing judgment she was able to prevent a medication error before it reached the patient.
As her direct supervisor, I always find Vivian very thorough when providing patient care, even during crazy busy times, she will pause and do her critical thinking, make sure it’s safe and then proceed. That’s why she’s being awarded for many Great Catches.
Vivian had a patient that had a high dose of aspirin ordered; the patient was here for a cardiac condition and Vivian just had a feeling that something was wrong with the dose. Vivian called the pharmacy and verified that the dose was appropriate and not above that max dose per day. Although the dose was within the limits per pharmacy Vivian was still concerned and escalated the concern to the attending and cardiologist. In the end, it turns out that the patient was only supposed to receive 1/2 of the ordered dose 2 times a day rather than the full dose 3 times a day. Due to Vivian's good critical thinking and nursing judgment she was able to prevent a medication error before it reached the patient.
Vivian continuously improves herself, and she’s a great resource person to ask questions. She voluntarily got involved in a hospital-wide project “Code Lavender” and is the unit champion for it, providing education to staff. Vivian is also our skincare expert, who always helps M coordinate skin audit every quarter. Recently 3 East received a patient from the 2nd floor, whose daughter had a bad experience and complained about many things and requested to be transferred to a different floor. It’s a very challenging situation indeed. The charge nurse and I personally did a lot of investigation and then gave Vivian a heads up on what was going on. Vivian was confident and formed a plan. She took this difficult assignment without hesitation, carefully got the room ready preparing for the patient's arrival. Vivian is such a great asset to MemorialCare.
