Ms. Campbell, eager to help discharge the patient, thought ahead and asked the team to place orders for discharge meds early to have them filled and ready on time.
Ms. Campbell is an extraordinary nurse and discharge nurse on the Med-Surg units. Recently, Team A had a patient pending discharge home to Victoria, TX, who did not have a ride home. I arranged for the Veterans Transport Services (VTS) to give him a ride home; however, he would have to be ready by 1300 and waiting in front of CLC. Ms. Campbell, eager to help discharge the patient, thought ahead and asked the team to place orders for discharge meds early to have them filled and ready on time. One of the meds ordered for the patient ended up needing pharmacy drug request approval, and she reached out to our team resident to make sure the consult was placed. Even after the consult was placed, she re-engaged the pharmacy because the medication was still pending, ensuring that they were aware that the patient had to be discharged by 1300. She was able to get the pharmacy to fill the medication stat once it was approved, she wheeled the patient out in front of CLC, while a CNA picked up the medications from the pharmacy right at 1300. The Veteran was discharged home on time and did not have to stay another day. Ms. Campbell goes above and beyond to ensure the Veterans’ needs are all met. She also exhibits the ICARE values with the care and commitment to our nation's Veterans. The case managers are so grateful to have the discharge nurses like Ms. Campbell and appreciate their hard work!
