For the family and caregivers of our veterans, the possibility of falling through the cracks in their healthcare continuum could be devastating and is unacceptable. Continuity of care is a major goal of all staff at the San Francisco VA, which wouldn't be possible without compassionate and skilled Patient Flow Case Managers like Wendy Li. Wendy recently went above and beyond, as she often does, and a grateful family member wrote a long letter of appreciation for her attention and professionalism:
"Wendy Li was a member of the team who helped us get our plan in motion and followed up with us more than once after my dad was discharged. She not only worked hard to ensure that we had all the necessary connections and contacts at the VA in Portland, but she wrote to us several times after we left to make sure that we got to Portland safely. It was those little touches... the basic niceties that are so often overlooked these days... that prompted this letter."
