A patient was discharged with five new meds as a Spanish-speaking illiterate patient. Yum jumped into action, gathering resources such as pictures of pills with the pharmacy, working with Med to Beds, and calling anyone to help translate in-person. She was able to reach the patient's housemate, who read Spanish, and have them come in. She got an in-person interpreter to go to the patient’s bedside. It would have been easy to go ahead and discharge the patient with a video interpreter. Yum took the extra step to help the patient be successful after being discharged.
