Tori was working with a patient in MSICU who was transferred to TGH for a surgical evaluation. Mr. S's procedure was canceled multiple days in a row and when Tori came on shift Thursday she called the surgical team numerous times to ensure the patient was seen and treated. Unfortunately, the patient received a terminal diagnosis after his procedure and was left without any further treatment options. Tori consoled the patient and his wife and let them know that she would do everything she could so that he could make it back home. Tori contacted the hospice team throughout the day Friday to help the patient get his dying wish to go home. Late Friday the patient was seen by the Lifepath hospice team but was informed he was not a candidate due to his residence in Manatee County. At 5:00 pm Tori began calling the Manatee County Tidewell Hospice and was able to get an intake RN to start working on Mr. S's pre-authorizations. Tori apologized and let the patient and his wife know that she wasn't able to set-up the discharge that night, but she was going to make it happen the next day. Tori offered to come in and help the next day, the unit was short and she didn't want to see her patient fall through the cracks. The next morning on Saturday Tori worked with Tidewell, she obtained necessary orders, obtained needed supplies, and provided specific trach care teaching to expedite her patient's discharge. The patient's wife said that Tori was "an angel sent down to watch over them".
On Saturday Tori's other patient in the ICU coded, the team performed ACLS interventions for almost an hour and the patient passed away. While Tori was coordinating care with the Tidewell hospice team she also took the time to call the deceased patient's family to offer her condolences and to see if there was any way she could support them. Tori consistently provides her patients with world-class care, she truly embodies the values of a TGH and a DAISY RN. Tori said Mr. S. left with a smile on his face, knowing he would be able to pass peacefully in his home. His last words to the team were "Thank you, I'm going to make sure the right people know about you." The patient's wife said she will never forget how hard Tori worked for her husband so he wouldn't fall through the cracks and so his dying wishes were met.
