Team members on 4South manage the care of complex patients and families in extraordinary ways. 4South receives patients debilitated from their baseline with increased confusion and challenging behaviors. Within a short timeframe, the patients are up, out of bed and interacting in a common area. Though the patient may still be confused, the team works diligently to provide the least restrictive environment in the most therapeutic way. In close collaboration with Social Work, Case Management and Therapy Services, they treat and care for patients with dignity, compassion, and grace.
An example of this extraordinary teams' abilities is when a patient, who had been hospitalized and debilitated to being bedbound and non-ambulatory for over a year, transitioned to 4South, was rehabilitated to his prior independence and discharged. The patient was challenging due to their demeanor and social issues, but the team worked every day to return the patient back to a new normal baseline.
Working with physical therapy, walking goals were set and upheld every day (the patient was required to walk the length of the hallway multiple times a day).
Working with Social Work and Case Management, behaviors modification goals were set; the patient was required to sit in the common area for all meals and remain appropriate toward staff and other patients.
The nursing team continued to modify the patient's goals in order to return to independence, through challenges, setbacks and managing multiple other challenging patients; the team never faltered.
The team members on 4South continue to show resiliency, act with intent and courage in everyday events. South Shore is proud to have care for this diverse group of patients and proud of every team member who contributes.
