Cheryl Griffith, MSN, RN, our Stroke Coordinator, has led Hahnemann to achieve incredible outcomes from her innovation at the front line.
To improve compliance with the stroke education core measure, Cheryl implemented Target Teach to help patients and families understand why stroke occurs and to develop an action plan for secondary prevention. Since Target Teach this core measure has maintained 100% compliance. Cheryl also provides stroke education to all staff house-wide.
Cheryl ensures all patients discharged have a follow-up appointment scheduled, and receive frequent phone calls by her to check on how they are progressing post-discharge.
Her incredible efforts have led Hahnemann to receive the Gold Plus Award for Stroke in 2015! Cheryl provides stroke-related care with passion, enthusiasm, and patience. Cheryl exemplifies a true DAISY nurse!
