This Team has juggled several projects and changes in recent years.
The Medical Management Team at NWCH is comprised of both RN Care Managers and Social Workers. I am proud to lead this team of collaborative individuals.
The Team has worked very hard, taking on multiple initiatives that benefit patients and their families, as well as the organization.
The most recent project they have been working on is a Bundle Payment Program, where Inpatient Care Management partners with the CDI team to identify patients by DRG, and Care Management links with OP Care Management through e-mail correspondence in system. With this partnership, one of the goals is to drive down readmissions.
Other initiatives that the team actively participates in are: ethics committee, EMMI training, COPD education Collaborative, breastfeeding taskforce, Niche accreditation, DDSO collaborative, Catholic charities prescription drug program, and Newark-Wayne Joint Center. The team does a lot of heavy lifting behind the scenes to drive change and improvements, working together and with other departments to solve challenging and complex issues. The Medical Management Team has many attributes, but 'teamwork' is by far the most valuable.
