Laura Brown was the RN case manager for a patient who was at our hospital for over 60 days. She tirelessly worked toward the safest, most appropriate discharge for this patient. She participated in care conferences, partnered thoroughly with the interdisciplinary team and went to bat for this patient. The patient wasn't able to return to her assisted living facility and needed specialized care due to her feeding tube. When the patient was refused admission at one facility, Laura went above and beyond reaching out to the intake staff. Laura provided the facility with detailed information pertaining to the feeding tube and its care. Ultimately, the patient was discharged to the facility that had initially refused - thanks to Laura's persistence and willingness to go the extra mile for this patient. Laura's dedication to this patient and her family resonated with her colleagues and facilitated the patient's transfer to the best level of care so the patient could continue her healing process in safety and with respect. This is but a single situation where Laura shone as what it means to be a DAISY nurse.
