Tina Truong, who works in Transitional Care is a treasure. There is no other word for her. As I serve as a court appointed medical decision maker for vulnerable adults, I am accustomed to having to overcome barriers to get people the care they need. I have grown to expect it.
Ms. Truong shattered that perception for me in November 2017 when I first met her. She worked to get a medically fragile, indigent man represented by our office. She told me she had known this patient for a year. She also told me she had spent months working to get his Medicaid approved since the patient's dementia and mental health conditions made getting accurate information from him challenging. The patient was also a refugee from a war-torn country who had experienced intense trauma and loss. Ms. Truong understood that and was able to work with this patient in a way that made him feel safe and care for.
She also did an excellent job assisting me with care coordination to move from the hospital to primary care at Richmond Clinic and psychiatric care at the Intercultural Psychiatric Program. Both of these settings also provided excellent care that reduced symptom distress ad they were able to provide care in a person-centered way that improved his experience.
The best part about working with Ms. Truong Is that months later, I had a challenge trying to do something for this patient and I took a chance calling her for help because I thought she might have the expertise, and I did not know who else to call. Ms. Truong remembered me, this patient, and stepped right up to get me the information I needed to move forward. THIS is the kind of care I wish every client of mine received every time they accessed health care. Tina Truong's methodology should be the standard for training your employees.
