I was taking care of a patient in the ICU who was requesting hospice. Polly was consulted and planned a family meeting to discuss his plan of care. I explained to the patient that his brothers were arriving shortly to discuss palliative care with the staff. The patient then told me he needed two things before he died. He stated he wanted me to "send a message" to the dialysis center and thank them for the excellent care that they gave him. The second request was for a quart of buttermilk from WalMart. He told me he was a "buttermilk-a-holic" and that it was the best thing in the world. I told him these were very reasonable requests and I promised him I would get him some buttermilk and speak to the dialysis center myself. The patient's blood pressure began dropping even prior to his family's arrival. Polly was on the unit waiting to meet with his brothers and I expressed my concern that he wouldn't survive long enough for me to get his buttermilk. Polly offered to run to WalMart and get the brand he had requested. She arrived back shortly with two quarts of buttermilk. When she put the straw to his lips he began eagerly gulping and broke out into a big smile. Polly is always wonderful to work with but today I was especially thankful for her generosity and kindness to help fulfill my patient's final wish.
