Jacqueline provided excellent family-centered care. We saw her briefly in the clinic, and I asked her about the patient portal. She said that she didn't know the process for registration, but she would find out. A few days later, she called me and asked for my e-mail address so that I could be sent a registration code for MyChart. She followed-up on my inquiry to find the answer and made sure she connected back with me. This excellent follow-up is above and beyond the care that is expected. It is so important for us to have access to our health record, and Jacqueline made sure it happened for us. What I know through my work in Patient Experience, and my lived experience as a caregiver for my son, is that the little things can make a huge difference in the experience of care. It does not take grand gestures - saying hello, being friendly, following-up with patients, asking how we are doing/feeling, asking what our concerns might be. I feel that Jacqueline demonstrated these wonderful qualities of patient-centered Care in her interactions with us.
