Ms. Lin Yuan An has always considered nursing as her interest and a career. She has 23 years of nursing experience and is currently working as a diabetic case manager at a rural clinic of the Min-Sheng Health System. After graduating with a bachelor degree, she worked mostly on the medical unit and medical critical care unit, prior to working as a case manager.
Since becoming a diabetic case manager in 2011, she has enrolled two hundred diabetic patients to follow the diabetic clinical practice guidelines; there were none before her arrival. She was also able to achieve 90% compliance of diabetic patients getting blood sugar control and Creatinine testing when due.
Ms. Lin demonstrated tremendous patience while teaching elderly diabetic patients to accurately measure insulin for self administration. She always makes sure that the patients or family members have the necessary competence before releasing them to self administer. She follows up on patients when they don’t show up for the scheduled appointment, either by phone or visiting them at the hospital when necessary. She took personal interest in working with the family members of diabetic patients and encouraged the engagement of extended family members; she has held family conferences when necessary, and provided emotional support to them when they are discouraged. She was able to obtain donations for blood sugar testing machines when patients were not able to afford one.
As a case manager, she has achieved the roles of being an excellent listener, teacher, negotiator, and of providing appropriate individualized treatment plans. She has gone beyond just performing routine work; she demonstrated dedication to her work, but also dug into the psychological, social, and economical aspects of patient needs, ensuring that the patients receive comprehensive care.
Ms. Lin is a cancer survivor who went through surgery and chemotherapy. She understands the anxiety, fear, pain and the burden that a family goes through. She is very motivated, and believes in helping in the prevention of complications, through educating patients and their families, the understanding of self management, and improving quality of life. She has received many compliments and the appreciation from her patients, and she was chosen as best nurse three different years and as best employee in 2012.
