Jennifer demonstrated excellence in meeting the physical and psychosocial needs of her patient who was hospitalized for a week. The patient experienced communication difficulties and challenges with pain management with her medical team after surgery. Jennifer intervened and advocated for her patient to maintain the PCA to effectively manage her post-operative pain. Jennifer facilitated coordination of care when the family medicine group attempted to discontinue pain therapy, which was to be managed by the surgical team. Jennifer and her peers worked to involve the patient in her own care management by partnering with and involving the patient in the decision process. Jennifer had excellent listening skills and heard the distress the situation was causing her patient. Jennifer remained compassionate and supportive and helped to intervene. Jennifer supported her patient and family and deescalated the unsettling situation. Jennifer turned around an unfortunate patient experience and helped the patient relax, so she could without hesitation trust the entire team of caregivers, recover from surgery, and progress to discharge.
