Marlene was caring for a 25-year-old patient who was very emotional and upset, which was increasing her MS symptoms. The patient was trying to contact her primary care physician without success the past few days and when her symptoms were unmanageable, she came to the emergency room. The following morning, the physician was discharging her to home and the patient was extremely upset. She was also having pain and the doctor would not reorder additional medications. The patient was afraid to go home and have the same thing happen again. With the patient's permission, Marlene called the patient's primary physician's office and provided them with the circumstances of patient's admission and concerns and asked if the patient could be seen as soon as possible for continuity of care and medication adjustments. The office arranged for the patient to be seen the next morning for follow-up and medication reconciliation. The patient and her husband were very appreciative of Marlene's going the extra mile to help her patient have a safe discharge.
