Carol has been going beyond her call of duties as an RN Case Manager. Not only do her co-workers admire her hard work and kind caring heart towards our patients but the patients themselves really speak highly of Carol Merwede. Carol is one of the unsung heroes of Population Health Care Management. Even with Carol's complex caseload, she has taken the time to give patients and their families peace of mind knowing that she is a tremendous support and resource for them. Much of the time, there are constant changes with patients' meds, hospitalizations, disease management, and care needs and Carol is omnipresent yet shy's away from taking any of the credit for playing such a critical role in coordinating each patient's care. Carol is organized, efficient, multitasks and streamlines her duties all while navigating through Epic upgrades and changes. Carol ensures the details have NOT been overlooked providing such compassionate, patient-centered quality. Her vigilance and dedication have greatly reduced hospital admissions and SAFE Events and empowered patients with managing their care. Whenever there is a challenging situation, however complex, Carol will be at your side acting as a problem-solving team member with a helping hand - Epic SuperUser and strategic planning with streamlining workflow. Carol also reassures family members of patients with positive updates about their loved ones. She is an integral part of Population Health, adored by the patients and well respected by her peers. She is devoted and works long hours, always willing to help everyone and maintains a positive and motivated disposition that is inspirational.
When at work, her goal is to improve the quality of a patient's life one moment at a time. She goes beyond extra miles for the patients and her co-workers. As her teammates, we have confidence in her to collaborate on cases and follow strategies for best practices improving outcomes.
Carol's work ethic, sense of duty and responsibility, physical stamina, sense of humor and emotional flexibility are commendable. She does not shy away from new technologies and department changes but instead embraces them enthusiastically as her greatest desire is to help patients clinically and emotionally. Carol has excellent communication skills and thrives in meeting new challenges. She serves as the ultimate patient advocate. She is able to establish a level of trust and cooperation from our special patients who are at times difficult to handle. She offers creative approaches to overcome barriers. She is very tuned into her patients and takes the time to listen to their concerns and to answer their questions. It is a joy working with Carol because we have witnessed her excellent nursing skills and compassionate attitude. Finally, Carol is an extraordinary person and clinical advocate who epitomizes nursing complex care management at its best. We have witnessed Carol's excellence in action for many years and she is exceptional and a true DAISY Nurse.
