I just wanted to take a moment and recognize a wonderful nurse from 4 South. Today has been a very challenging day. Care coordination Department is running with short Social Worker staff, however, I am glad to work with such a proactive team player as Vita. We had a patient with a discharge order for a Social Worker after prescriptions were filled. Vita took the prescriptions and faxed them directly to the pharmacy helping our Social Worker to expedite the discharge. Another patient attending placed orders for discharge if arrangement completed by SW too. This case was referred for SNF placement. The Coral Reef facility gave a hard time on acceptance because the patient was still on droplet precautions. Once again, Vita called the attending and the patient was cleared safely from droplet precautions and accepted at the SNF to be discharged today. Another patient required coordination from a Social Worker for a roundtrip PET scan. We were waiting for an MD call for preliminary biopsy results and diagnosis. Finally, the patient was confirmed to have a cancerous condition, and Vita documented that on the PET scan so that our documents now show our conversation with the MD, helping us fill the information needed. Thank you to Vita again for your proactive attitude, for your professionalism and always good mood to help us in our hard and challenging work. Thanks for understanding our role and collaborating to ensure a high rate of patient satisfaction. Vita is amazing.
