We've had a patient on our floor for a few weeks who has been directed/suggested to become a hospice care patient. Cherrelle has been caring for this individual for 10 days. Each day during interdisciplinary rounds Cherrelle states the patient is doing better and is NOT appropriate for hospice. With her dedication to the individual's wellbeing, the patient is now eating by mouth and up in the chair in the hallway. The patient is now being discharged to a Skilled Nursing Facility stay and will soon be back with his family and society. This is exceptional continuity of care. Each day Cherrelle chose this patient who is a complete patient with a trach, colostomy, foley, central line, sacral and abdominal wound, and dobhoff. He now eats on his own, no foley, trach or central line.
