Andrea goes over and above every day as she calls patients who have been discharged from the hospital with chronic conditions. These patients require intense case management and coordination of care to ensure that they have everything they need from medications, home health, a primary care provider, oxygen, nebulizers, and follow-up appointments. As she works to ensure the patient has the necessary items in place to avoid hospital readmission, exacerbation of their illness, questions, or complication, she takes the time and care to follow up and provide the resources needed. Andrea will often start very early in the morning, work well into the evenings, and even weekends to ensure that the patient has everything they need at home. She doesn't just call the patient once, she follows up for at least 31 days as events usually occur during that time where the patient has questions or issues that need a nurse to resolve. She is the true meaning of a dedicated nurse.
