I would like to nominate Valery Li for the DAISY Award. Valery has a keen eye for detail and easily detects changes and issues that we encounter on the unit. She recently encountered a problem with the primary IV tubing. She noticed that it happened every time she primed an IV in recent months. Then, she randomly surveyed nurses on the unit to see if they were also encountering the same issue. She discovered that everyone was having issues with the primary tubing. It would take multiple attempts, opening up multiple packages of IV lines to obtain a patent and operational IV tubing. She emailed me and the nurse educator about the issue and after the nurse educator surveyed other units, they summated that the problem was the product. The manufacturer was contacted and admitted that they changed how they made the tubing which made it harder to prime the tubing. Now, they will be sending an entire new stock of primary IV tubing in the later parts of June.
This was a recall of a product caught early which could have cost the facility thousands of dollars in losses due to multiple attempts in priming one IV bag. This was outstanding work of the staff nurse who spoke up and made the finding known to management, nursing education, SPD and the distributor.
