The patient's son stated that he noticed his mom was more lethargic and seemed to be having difficulty answering questions. The patient was unable to lift her left arm and left leg. PT came to evaluate the patient and noted she could not put any weight on the left side. Prior to admission the patient lived with her son and was able to walk independently. The patient was initially diagnosed with dementia. Three days later, Libby Jones was assigned to this patient. During her morning assessment, she noted the findings and immediately did a MENDS exam. She called the on-call physician and got an order for a CT. She initiated the stroke protocol. I visited with the son and he stated that Libby saved his mother's life. The patient had been on blood thinners and Libby stopped those immediately. She continued to follow up with being this patient's advocate. I have observed her with many other patients and she always is an advocate.
