Jan O’Reilly provided exceptional nursing care management to a patient that had multiple healthcare and psychosocial issues by spending numorous hours of her time to coordinate, teach, listen, and reach out for community resources. Many of the hours were unpaid and she personally committed herself to assisting this very difficult homebound patient's needs.
