A 31-year-old patient came to the ICU as an emergent upgrade for shortness of breath. During this time the patient was placed on a BiPAP where he stayed for several hours throughout the remainder of the night shift, breathing 40-50 breaths per minute. Intubation was inevitable, as Jacob realized immediately as he assessed this patient at the beginning of his shift.
Through the chaos of receiving and completing orders from the hospitalist, placing consults for a pulmonologist and other ancillary specialty providers, this patient had one request, which was to call his one-year-old son to wish him a Happy Birthday. Even though Jacob was incredibly busy preparing to intubate this young man, he knew how important this was to his patient and stood by his side while he made, what turned out to be his final phone call to his son.
I watched Jacob over the next three shifts care for this patient and family better than I could have ever imagined. After this patient was intubated, Jacob took the time to gather the family to discuss the plan of care, such as lining, imaging, medications, and therapies that the patient would be receiving over the next several hours. He managed to field questions while he allowed the family to stay bedside for as many of the tests as was possible.
Through speaking with the family at the bedside, he learned that it was the patient's mother's birthday the following day; she lived out of state and would be traveling to Phoenix on her birthday. Through the mayhem of traveling to CT, coding the patient, assisting with several bedside procedures and starting CRRT, Jacob still managed to make time to pick up a birthday balloon and stuffed animal for the patient's mother on her birthday.
Unfortunately, even after all heroic measures were done, this young man was still taken from us. I believe that Jacob made an incredible impact on the patient and his whole family in this devastating time. He is the epitome of a bedside ICU nurse; professional, thoughtful and holistic. I am proud to call Jacob my coworker and to have the honor of working alongside such an outstanding nurse in the MICU.
