Integrity
Dee Diaz has demonstrated her integrity to me in several ways:
1. She called my home to report on a situation that I had called her in regard to my diabetes. I was not home at the time she called. My wife asked her for the information and she explained to the wife that because of privacy laws, she could not share it. That was proof to me of her knowledge of the privacy laws was indeed very complete. My wife also commented on her ability to convey the answer very clearly of why she was unable to share the information and in a very polite manner.
2. When I called Dee, she always returned my calls and communicates the information in a very clear and concise manner.
3. When I meet with Dee for an appointment, she very clearly writes down any medication changes that would help my well-being.
Commitment
Nurse Diaz demonstrated her commitment to me, a former Army soldier, the VA's Mission of sincerity in many ways. A real meaningful example occurred in July of this year. I was supposed to call her back on a medical issue. I did not do this in a timely fashion, so she called me and said, "how are you Jack - I miss you." That one blew me away and will remain in my memories. Nurse Diaz cares about me.
Advocacy
To be an advocate is to help and support a cause or person. In my case nurse Diaz suggested to me that just because I am 85 years old (close to 86), that I am not really old. I work out with weights at home and in warm weather months I ride my bicycle at least 24-30 miles a week. She said, "you should be proud to do those things and feel good about yourself." I have followed her directions and have improved my body strength because of her support!!
Respect
Nurse Diaz has never downgraded my mental or physical assets. Her smile, encouragement, sincerity have helped me to keep a positive attitude I shall always have kind thoughts of her because of that.
Excellence
This word describes the fortitude and moral advocacy at nurse Diaz practices. She has "excellent" patience and has been especially kind to me. Yes, I am old, but she helps me to overlook that difficulty and encourages me to do the best that I can with the physical and mental assets that I have truly been blessed with.

I have enjoyed writing this nomination for the DAISY Award for Extraordinary Nurses on behalf of Nurse Diaz. I would also like to say that I am very thankful for the "Primary Care Team" and has been assigned to me. Flora Cody, LPN as well as Lana Buttke, NP, and Dawn Sitter, MSA, have also encouraged me to do and be the best that I can be. "Thank you" to all at the Union Grove Primary Care Clinic."
