I have had the privilege of watching Hannah grow in her nursing practice over the last four years. Hannah is a tremendous advocate for her patients and families, consistently going above and beyond to provide compassionate care.
Most recently, I was able to watch Hannah tackle a complex discharge that was riddled with issues that had not been resolved the evening before an early morning departure. Hannah took all the challenges in stride and worked tirelessly to ensure the patient and family felt supported on this unknown journey. At one point, Hannah was holding two phones: speaking with people about lodging arrangements, making plans with social work, and arranging medications to be picked up by the family. Throughout it all, Hannah maintained her professionalism and compassion for the family, providing multiple opportunities for reeducation when the family was not comprehending the material.
I witnessed Hannah walking alongside the patient asking, "How are you feeling about discharging tomorrow?" Hannah provided space for the patient to express their fears and anxieties over leaving the hospital system that they had come to know very well. Hannah worked extremely hard to tie up any remaining loose ends to set the next shift up for success, and to ensure the discharge was successful for this patient and family.
