Ashley was caring for a patient admitted with chest pain. The patient had a history of cardiomyopathy, CAD with stent placement. There was also a history of chronic alcohol and drug abuse. Some of his complaints were written off as being related to his addiction and nonadherence to his treatment protocols.
While speaking to this patient, Ashley was able to determine that one of the reasons he did not continue with his care was that he was essentially homeless and without transportation. Reaching out to case management, Ashley contacted a program in place through the fire/paramedic department providing transportation to follow-up appointments for those in need. Prior to discharge, Ashley had made contact and set up the appointment for this patient. By working through bias and setting up these appointments for this patient, she offered him a better chance for a positive outcome. I feel she went above and beyond in this case.
