Anne took it upon herself to prepare his medications weekly, provide continued education to the patient on the importance of medication compliance
We had a 54-year-old patient, who was struggling with blood pressure management and medication compliance. This patient was on the transplant list and the transplant center passed him up for a transplant in 2020 due to non-compliance with medication and uncontrolled blood pressure. Anne took it upon herself to prepare his medications weekly, provide continued education to the patient on the importance of medication compliance, and followed up with him each week to ensure he understood the importance of taking his medications and that he was educated and able to do so independently. Initially, Anne would have the patient bring his medications and pillboxes in-center and she would put his medications by dose in the individualized pillboxes each week. This patient then progressed to doing this independently and bringing the medications in weekly for Anne to continue to review. This patient’s compliance with his medications improved, and his blood pressure became stabilized. The greatest success of this story is this patient was able to receive a kidney transplant on 2/16/2021 due to his increased understanding of the importance of medication compliance and his controlled blood pressure. Anne went above normal medication reconciliation, she assisted with identifying cultural barriers, and overcame language barriers to ensure continued improvement for this patient.
