Her consistency and dedication have allowed Alma to build trust with our patients.
Along with the numerous asks and demands of COVID-19 as well as daily patient care, it is easy to overlook the things that happen on a daily basis. I would like to recognize Alma Diaz because of her out-of-the-box thinking when it comes to patient care. She takes the time to truly get to know our families and understand their different dynamics in order to best serve them. She goes above and beyond, by not only working well with our team and building positive team morale but making it a positive environment for our patients.
She has recently been promoted as our Clinical Nurse manager and since being promoted we have been short-staffed due to the demands of COVID-19 and she has never failed to provide coverage and step in as many days that may be needed to ensure safe and efficient patient care. Even with working five to six long days per week, she is always in a great mood and has an amazing team player mentality. She thinks proactively to ensure problems do not arise and patients are in a happy and safe environment. This facility would not be the same without her and has been successful due to her hard work and dedication. She continues to stand out to both our team members, physicians as well as to our patients as someone that they can all go to for guidance, support, and optimal patient care. Her continued commitment to our patients has not gone unnoticed and has encouraged her team to step up as well.
An example of her willingness and extraordinary focus on the patient experience can be seen in the way she cares for her patients. Specifically, we have an elderly patient who has been at our facility under Alma’s care for many years. Alma has taken the time to build rapport with both the patient as well as the family by following through on her commitments as well as ensuring clear communication between the patient and the nephrologist. Her attention to detail helped this patient stay out of the hospital, during a very critical time. By working with this patient and family, medications were adjusted and items were not missed thanks to her focus and tenacity to keep our patients out of the hospital. Her consistency and dedication have allowed Alma to build trust with our patients and get their buy-in to be adherent to their dialysis routine. Alma takes the time to call the patient’s family with updates and makes sure she follows up with timely commitments, which has proven to this family that she truly loves being a nurse and gets fulfillment in her daily responsibilities of providing dialysis care. She gives the patients everything she has and more and the patients and families truly confide in her.
