She persisted and eventually was able to get the patient placed at a nursing home late afternoon.
Dr. B noted that they both showed up for their shift one Sunday morning and there was a patient who had been discharged the prior shift and was waiting on a ride. EMS relayed to Diane that they had been to this gentleman’s home almost daily recently for falls and lift assists due to a chronic progressive medical condition. He had multiple physical/mental ailments and she was concerned for his safety at home. He lived alone with no family in town. We called contacts and no one was able to come and get the gentleman. It took two of us to lift/transfer him to the toilet, etc. Physical Therapy agreed he wasn’t physically able to care for himself at home and the patient agreed to a nursing home placement. It was a Sunday and Diane had no support to arrange this, so she spent most of her shift making phone calls, filling out paperwork, and tackling roadblocks, mostly related to insurance. She persisted and eventually was able to get the patient placed at a nursing home late afternoon. She went way above and beyond her job duties for this gentleman.

This is Diane's 2nd DAISY Award!!
