They round on the Resource and Floated staff as well to ensure they also have the tools they need to provide the best care possible.
The ED is stretched daily with ED volume as well as an increasing number of boarded patients as admission rates rise, an overall increase in Trauma Volume, and as a receiving center for transfers coming to the facility for specialty consults. Without teamwork, they wouldn’t be unable to function. Each day begins with a shuffle to determine the sickest patients and prioritize their care. Shane and Rachel, the TC, can often be found managing the waiting room and pulling patients to Focused Care areas to simply decrease the waiting room times for less acute patients. They creatively use the hallways, the trauma bays, and any unused space to proactively manage the patients and to show the support of the team. Most weekends Shane or Rachel are here as well to support the staff and fill in as needed especially between 15-2300. They gladly step into a charge assignment, manage the sickest patients, transport, or wherever the team needs them to do. They coordinate with the Bed Tower to prioritize the sickest and longest stay patients out of the ED to an inpatient bed where we know they receive much better care. They round, provide gentle coaching, all while holding the ED staff to the highest standard Sentara sets for us.

We have received many thank you letters from patients and families pleased with the care in a very busy Emergency Department. The ED is an extremely difficult place to maneuver and to work. Most days, assignments expand far beyond inpatient assignments. This is done with an eye to managing the WR and decreasing the overall length of stay in the ED. Shane and Rachel know starting their care as soon as they walk in shortens the time needed on the back end for additional testing. The leadership rounds consistently and continuously on staff, providers, and the patients to help them understand the goals for the day and to ensure we prioritize safe care and management of the patients waiting above all else. We know from the data the ED contributes minimally to HAPI, CAUTI, and CLABSIs because of the proactive documentation and 100% follow-up on any Foley and Central Line placed in the ED. They promote the participation by the staff in data collection and peer coaching where possible because they understand that a team accountable to each other is much stronger and has better outcomes.

Additionally, there is a strong partnership and daily conversations with our ancillary colleagues to review any barriers to LOS. This includes Radiology and the Lab as well as other disciplines as needed. Leadership rounds with the providers to help determine the priorities in patient movement and moving patients to more acute areas as needed. They round on the Resource and Floated staff as well to ensure they also have the tools they need to provide the best care possible. Although wait times are often much longer than we like, leadership calls every single patient complaint and tries to resolve it even if that means bringing the patient back to complete care.

Dozens of patients were brought back for repeat Covid tests whose samples were unable to be processed or simply did not make it to the lab timely. Each of those patients was offered the opportunity to come back free of charge to have the test redone at their convenience. Another example is sending staff to CIT training to learn more about de-escalation. This class was organized in conjunction with local CSB and the police department and focuses on scenario-based events that most often occur in our Emergency Department.

In addition to the training, the partnership with the police in having CIT officers stationed Friday-Monday in the ED to support the staff has been very well received by the staff and increases their support of their leaders. This is a partnership with the Norfolk Police and funded totally by the police and Grant funding through CSB. This also frees up Security to respond more quickly to other areas of the campus requiring their expertise.

Additionally, Shane and Rachel participated in the planning of the Flu drive in 2020 and delivered Flu Vaccines to over 150 people in the community. This is the second year in a row the ED has participated in this event.
