Aimee provided compassionate care to the patient until she was safely transported to the hospital.
It is a pleasure working with Aimee Faulhaber Full Time Case Manager for Team C63, who joined the VNA Health Group a little over 1 year ago. She has many strong attributes and could have easily picked all of the characteristics listed. Since I was asked to pick 3 of Aimee’s strongest characteristics, I would say the ones best to describe her are; 1- delivers holistic care encompassing mind, body, and spirit, 2- collaborates with the individual patient and family, and fosters communication, 3- delivers competent and compassionate care. Aimee is a strong Case Manager who provides excellent care to our patients and takes charge of any given situation. She is very kind, patient, and goes beyond to make sure her clients remain knowledgeable, healthy, and safe at home. During this very challenging past year of the Covid 19 virus, Aimee easily adapted to our “new normal” way of delivering patient care. She was among the first nurses to be trained as a SWAT nurse for Team C63 and there was no hesitation in her decision to attend.

She received a call from an HHA, who called to report that the husband of Aimee’s patient was found dead in the yard. Aimee called me immediately to say she was on her way to the home, it is worth noting this patient was not due for a visit that day. Because the client had Alzheimer’s she was not aware that her husband had passed away. The aide had found the husband in the yard dead as she approached the house and found the patient alone in the home gratefully in a safe condition. This was a very difficult situation as the husband was the primary CG and the estranged daughters were not reachable. Aimee provided compassionate care to the patient until she was safely transported to the hospital. Aimee remained organized and calm throughout this entire scenario as we had spoken several times on the phone. In my mind, Aimee went beyond the call of duty that day. After a few hours once the patient was sent to the hospital Aimee continued to see the rest of her assignment going about her hectic day as a Community Health Nurse.
