Teri recognized that a barrier in the patient’s DC plans could negatively impact her health, and worked to prevent it.
Teri was caring for a patient on PCU. The patient has had multiple readmissions to the hospital for asthma complications. Teri learned that the patient was currently homeless and living in her truck (in the middle of the hottest part of the summer). The patient states she had been unable to get into a shelter. After listening to the patient voice her frustration and difficulties, Teri was very concerned that she would repeatedly be in the hospital if safe housing wasn't secured. Teri took the time to reach out to a contact she has with the housing authority. She was able to give the patient the contact information for this person in the housing authority, and she was approved for emergency housing. The patient was so gracious for this assistance. This is what compassionate advocation looks like. She recognized that a barrier in the patient’s DC plans could negatively impact her health, and worked to prevent it. Teri is an amazing nurse and a caring person. I am proud to work with her.
