Discharge Milestones is a multidisciplinary level process that Diane helped to lead to create a more efficient and effective discharge process for our patients.
Diane exudes positivity and is a highlighted and valued role-model for our unit on Level 9. She shares her wealth of knowledge through precepting students and new-graduate nurses and through her leadership in the Charge Nurse role. From the compassion she shows at the bedside with her patients and families to the behind-the-scenes work she has put her heart and soul into with process improvement through completing her Greenbelt Certification, it is a true understatement to say Diane is an asset to our team!
Diane has leveraged the knowledge she gained through her Greenbelt certification to help improve processes throughout the organization at Children’s Hospital Colorado. Discharge Milestones is a multidisciplinary level process that Diane helped to lead to create a more efficient and effective discharge process for our patients. Since going live, this process has helped to streamline the needs of each individual patient and make visible, with a checklist of requirements to all healthcare providers, what needs to be done in order to get each patient home in a safe and organized manner.
On a higher level, Diane’s work on this project is being utilized as a measurement system hospital-wide to capture discharge delays in real-time and bring visibility to the organization where patients are at in the discharge process. This information is utilized to better improve our patient flow hospital-wide. We feel that Diane upholds the mission and values of this institution, is very focused on family-centered care, and exemplifies role-modeling and kindness. Team Member Support Council is honored to recognize Diane as a DAISY Nurse!
