Knowing full well that this procedure and treatment was not performed here in the hospital, Mayra, being ICU cross-trained, took it upon herself to coordinate this event because she knew that this was what was best for the patient.
There was a patient who was ICU status in the DOU. At the time the DOU did not have a charge nurse and Mayra had her own patients. Because the patient in question was so unstable (proned maxed out on vasopressors maxed out on the ventilator) a decision was made to transfer the patient to UCLA for the placement of an ECMO line in order to provide him with ECMO therapy. Because the patient was so unstable there was no way that this patient would be able to be transferred to UCLA for the procedure. The UCLA doctors decided to come to the hospital and insert the line as well as initiate therapy prior to transferring to UCLA. Knowing full well that this procedure and treatment was not performed here in the hospital, Mayra, being ICU cross-trained, took it upon herself to coordinate this event because she knew that this was what was best for the patient. She obtained a list of supplies that were required and reached out to the House Supervisor and OR charge nurse in order to obtain the needed OR supplies. She established a time frame for when the UCLA doctors would arrive so that she could coordinate with our lift team staff to supine the patient and decrease the amount of wait time for the patient. The MDs arrived and she assisted with a procedure that she had never done nor do we perform in the hospital. The patient was stabilized and able to transfer to UCLA where the appropriate care could be provided. Despite the circumstances presented to her, Mayra was able to successfully coordinate the care of a patient who would have otherwise passed away had he not received this life saving therapy. I know nurses save lives every day, but Mayra REALLY saved this patient on that day. We need more nurses like her.
