Chris consistently shows compassion for patients by working with them patiently and taking the time to be reassuring and supportive to people who have limited supports.
There has been a significant shortage of nurses with homecare agencies which have caused some patients to not be able to get homecare services after discharge from the hospital.

We had 2 patients whom Chris went above and beyond to assist. The first patient was a new ostomy patient who was discharged to home and was not able to get homecare due to the RN shortage. The patient needed supplies and guidance about her ostomy care. I called Chris and made her aware of the patient's situation. Chris remembered meeting with the patient in the hospital and told me she gives her number to all patients that she sees while in hospital. Chris called the patient and guided her at home, contacted an outside agency to help with getting supplies, and was able to assist the patient over the phone by providing her support until she could get to her physician's appointment.

The second patient was sent home with complex wound care and could not be staffed. I made Chris aware of the patient and she called the patient multiple times and guided her until the patient was able to start with a homecare agency more than a week after discharge.

Chris consistently shows compassion for patients by working with them patiently and taking the time to be reassuring and supportive to people who have limited supports.

She shows she is an active team member working with inpatient staff but also was willing to work with the outpatient team with the wound care center and navigation to problem solve for these patients and other patients who have had a delay in care.

When I made Chris aware of the problem we were having with accessing homecare she immediately started to talk to me to problem solve and help find a solution. Chris called outside agencies and outpatient services to get ostomy supplies and wound dressings for the patients in need and was a good advocate for them within the community.

Chris is an expert in her job and is willing to go the extra mile to help patients and works with team members with ease in order to get the job done. She deserves to be recognized for her fantastic effort and care.
