Marcus demonstrated compassionate care when he went above and beyond to assist in a very challenging discharge plan. Due to the patient's age and housing status (homeless), the patient posed several challenges upon discharge. Once a discharge plan was in place, Marcus leaned in and was instrumental in making sure the patient’s safety was the primary focus. Marcus provided quality care when he helped the patient navigate the process including riding to the shelter to help her get the paperwork submitted. Marcus helped the patient complete a 10-page intake and ensured the patient's safety at all times during the process. Marcus, having an understanding of the importance of clear communication, made sure that proper information was given to the Social Worker who could "take over" the patient's care. Throughout this process, Marcus paid special attention to ensuring the patient was always safe and treated with dignity, kindness, and respect. He went above and beyond to ensure that a "warm hand-off' was made for the patient. This discharge plan could have not been successful without the instrumental role that Marcus played.
