The form Kristen developed was adopted into the workflow as a part of the expected change of shift activities at the facility to enhance the continuity of care for the patients.
Kristen is a constant source of knowledge and kindness in our cohort. She is empathic, calm, and always wonderful at helping her fellow student get through a rough day. During the pediatric rotation, we helped with health screenings at an elementary school, and watching Kristen interact with the elementary kids taught me to be a better nurse. She explained what was happening in terms the kids understood and helped calm the kindergarteners who were going through these screenings for the first time ever.

Kristin was assigned to a clinical site for a preceptorship for her Leadership clinical experience. The precepting nurses work one week on and one week off. Kristin noted that information needed for patient care continuity was lost from week to week and created a ‘change of shift’ report form to ensure important information was not lost and that patient care could continue seamlessly from one shift to the next. Kristen’s preceptor recommended her ‘change of shift’ form be adopted by the clinical facility.
