Ms. Chantal Valentin was selected for her extraordinary dedication, compassion, observational assessment, and collaborative skills. Nurse Valentin was caring for a patient admitted to the hospital Medicine unit with the wrong identity. She and the Social workers were able to resolve that issue of getting him identified correctly. However, that night Ms. Chantel Valentin went home watching the news (ABC Live) and came across a story of a family requesting help in finding their missing loved one (A professor from an Elite University). Ms. Chantel Valentin realized the identified picture from the television as one of her patients who she was caring for. She did not realize he was missing. She immediately contacted her Nursing Manager at the hospital and went into work. Nurse Valentin was able to collaborate and partner with the hospital, nurse managers, social workers, risk management, and hospital police in reuniting the patient with his family. The reunion was a very heartfelt experience as the patient asked his family, "What took you so long." He was able to tell his family he received wonderful care from the hospital and especially his nurse. It was an emotional moment seeing his family relieved that their loved one was safe and alive. Ms. Valentin is a true value to our healthcare system.
