Jenn is always finding a way to offer praise during visits which is very important to this client.
Jennifer Joy RN Collaborative Care Nurse joined the Community Health Team embedded in the Primary Care Clinic about one year ago. Her personality, past nursing experience, and skill set bring an enhanced ability for her to know how to deftly assess clients for safety knowledge deficits resource needs, and which members were vital to have on the care team. Jenn interacts with her clients with empathy honesty and clarity. Jenn care manages a 60-year-old male who has complex chronic conditions and has a high level of intensity that requires management. He has been case managed by many other nurses and social workers but due to his behaviors and severity of needs, no worker has stayed with him for long. When Jenn started she was assigned this patient and has helped to transform him, Due to the care and education she provides the patient's hemoglobin A1C dropped 1.4%. Closing his knowledge gaps gives him confidence and the ability to be active in his own care and planning. He understands why reducing his hemoglobin A1C is important. The results show how it makes him feel better medically and mentally. He is not in crisis mode every day. When we asked the client about his relationship with Jenn he talked about how she keeps him on track with his goals and holds him accountable. Jenn is good about helping him solve problems like getting the items he needs so that he is able to stay comfortable. She is always finding a way to offer praise during visits which is very important to this client. He states, "She is an excellent nurse, I have never met a better nurse!" Jenn is a strong advocate for her patients as she will do whatever she can to help them succeed. This not only translates to her clients but also to the care team of which she is a key member.
