Sedina always goes above and beyond trying to make sure that Home Based Primary Care gets the information needed to order services a patient might need prior to going home from the hospital. I had a patient home recently who needed multiple services and a great deal of follow-up care. Sedina would always send emails or instant messages throughout the day keeping me informed and up to date on the patient's transition from hospital to home. Because of Sedina’s hard work, diligence and effort we were able to make sure the Veteran had everything needed to go home within 24 hours.
