She met with the veteran five times since January 2022, with numerous calls to his residence. She worked tirelessly on his plan of care to decrease his edema.
Chloe has been working with a specific veteran on the improvement of edema and medication adherence for four months. She met with the veteran five times since January 2022, with numerous calls to his residence. She worked tirelessly on his plan of care to decrease his edema. The veteran started out with 3+ pitting edema, hypertension, and shortness of breath. Chloe worked alongside the provider to ensure medication changes/adjustments and teach the importance of adherence to nebulizer treatment and fluid restrictions. She educated, coached, and encouraged the veteran on dietary and lifestyle modifications to lose weight and improve his fluid retention.
The veteran and his spouse fell under hard times and didn’t have a vehicle, so Chloe placed beneficiary travel for each visit; community providers educated the veteran on the importance of completing an echocardiogram and Holter monitoring that are necessary to track progress. The veteran has now lost 25 pounds, improved his blood pressure to 120s/70s, and has no shortness of breath. He reports less stress and a new, improved outlook on life!
We are proud to nominate Chloe Linn, an exemplary employee, and nurse who provides safe, compassionate, timely service to our Veterans! Chloe Linn only joined the Muskogee Primary Care Team about a year ago. Since then, Ms. Linn has:
• Volunteered to stay after work and make notification calls for Covid-19 test results during the peak Covid time in 2022
• Volunteered and represented JCMVA PC at Wreaths Across America at the Fort Gibson National Cemetery this past Christmas season so that every Veteran is honored & appreciated
• Volunteered for and was trained to be a certified PACT orientation nurse
• Volunteered and is the MUS Primary Care super-user nurse trainer for ePerformance
• Stayed late to assist in ED with a Primary Care patient that needed monoclonal antibodies
• Utilizes eQM data to enhance time-sensitive safety needs for Veterans with high CAN scores
• Collaborated with the entire Primary Care to win the “Rooted in Strength” enthusiasm award
• Partnered with two other Primary Care nurses to project “Above & Beyond the Call of Duty” award to acknowledge staff who reflect exemplary efforts in their work to boost morale and express appreciation
• As part of her care, Chloe informs Veterans how to contact the Call Center and relay a message directly to her (their nurse). She quickly returns calls and has increased her team-patient satisfaction/SHEP scores
• Floated to ED on an exceptionally busy day to lend support when they had a staffing shortage
• Took the Two-hour training to become Patient Satisfaction Lead for MUS PACT
• Currently working on a PMHNP degree and aspires to practice at JCMVA
• Chloe is an employee who values people and integrity
