The daughter was really reluctant and didn't want to but Holley was persistent and got her to agree to call 911.
It was reported to us that the veteran was exhibiting odd behavior, confusion, dizziness, and decreased appetite, but PSM anticoagulation pharmacy was not sure what was going on. So they directed him to go to urgent care for lab and other workups, the vet agreed and his son was going to take him in. We could not find documentation that his son did indeed bring him into any urgent cares/ER for a workup. He was scheduled with us for mental health provider routine f/u the next day. I told Holley that morning that if he did not log onto the visit that a health and welfare check would be warranted due to red flag concerns reported from PSM anticoagulation clinic and no indication that he went in for evaluation.

At the appt time Holley logged in and the vet was not on for the intake. So she called his phone, and his daughter (with whom he has an estranged relationship with, so she likely does not know his current health status) answered the phone. She told Holley that she was there checking on him due to the VA calling numerous times with concerns. The daughter told Holley that while she was there her dad tried to get up and fell back and hit his head on the wall. She said that she was not able to get him up due to him feeling dizzy. Holley told her to call 911. The daughter was really reluctant and didn't want to but Holley was persistent and got her to agree to call 911. While they were waiting for the ambulance to get there the vet fell again and broke his clavicle.

Holley called to f/u and the vet was admitted to Mercy. Holley then checked to ensure that it was reported and documented that he was taken into the hospital so that payment could hopefully be authorized. Looks like the veteran has a broken clavicle and he will be getting a pacemaker placed, with likely rehab after.

Another example of Holley being an excellent nurse is Holley sat with a veteran for 30 minutes post-MH appointment until the DAV arrived to pick him up. The veteran did not seem safe to be attending the appointment alone, so she stayed with him to ensure his safety. She also provided him with some needed catheter care while they were waiting. Once the DAV arrived the veteran was not able to get into the van safely. Holley assisted him into the van and directed the DAV driver to have the care facility staff help get the veteran out of the van once they arrived back at the care facility.

Holley is always so on top of stuff with caring for the veterans! I love working with her because I always know that I can completely trust her follow-up and follow-through to meet the veteran's needs!
